Billy’s transition
Recently we supported Billy and his family to transition to CareChoice in home support in their large regional town. Billy is a young person with very complex behavioural needs and, while he was not able to continue to live in his family home, it was important to him that he would still be able to see his family regularly and stay connected to the community he had grown up in.
CareChoice Housing worked closely with Billy’s care team and family to fully understand his support needs and what home meant to him to enable us to secure the right SDA housing model in their area. Creating a detailed Personal Profile and Individual Care Plan allowed our Client Engagement Specialists to identify the skills and qualifications that would be required to ensure the best support for Billy. Targeted recruitment and training from his behavioural support practitioner gave his new team the skills to understand his support strategies and goals.
Four months on and it is wonderful to see how Billy has been able to have regular visits from his family and build on his involvement in his local community, including regular shopping trips.
